Community Nursing Care

At Bridgecare Disability Services, our Community Nursing Care service is dedicated to providing comprehensive and holistic care to individuals with disabilities in their home environment.

Our registered and highly skilled nurses offer a wide range of medical, rehabilitative, and supportive care to ensure our participants’ optimal health and well-being, all in accordance with the National Disability Insurance Scheme (NDIS).

Our services encompass a broad spectrum of care, including;

  • Wound care
  • Medication management
  • Chronic disease management
  • Post-hospitalisation care,
  • Palliative care, and assistance with daily self-care activities.

We firmly believe in adopting a person-centred approach, customising our services to meet the unique needs and preferences of each participant. In collaboration with other healthcare professionals, support workers, and participants, we work to develop a holistic, efficient care plan that respects the participant’s wishes.

The Community Nursing Care service at Bridgecare Disability Services aims to enhance the health outcomes of individuals with disabilities while maximising their comfort and dignity in their own homes. This service promotes independence, encourages self-management, and ensures continuity of care, ultimately supporting our participants’ overall well-being and quality of life, all under the framework of the NDIS.
